Chronic disease can be defined as any health condition that is present for the long term. Some common chronic diseases include diabetes, heart disease, asthma, arthritis, kidney disease, depression, lung disease and cancer, however, there are many more.

For patients living with a chronic medical condition, life can become stressful and may involve the need for some significant life changes. Our doctors can assist you by creating an individual GP Management Plan. A GP Management plan helps identify goals and puts in place a schedule of health consultations with your doctor.

Chronic disease management also often involves care from other health professionals such as nurses, dietarians, physiotherapists, podiatrists or other medical professionals.

Our doctors may initiate a Team Care Arrangement to eligible patients, to enable Medicare to provide you with subsidised allied health visits, usually five across a calendar year, to assist you in achieving optimal treatment options.
