Again.<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">She stood in her bathroom, heat pack pressed to her abdomen, trying to decide if she could make it through work.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Her period wasn\u2019t just inconvenient. It was overwhelming.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Heavy bleeding. Sharp, dragging pain. Exhaustion that clung. <strong><em>Bad periods<\/em><\/strong>, she\u2019d been told. Wait it out.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">So she did. For years.<\/p>\n<\/div>\n<\/div>\n\n\n\n<h2 class=\"wp-block-heading\">What is adenomyosis?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Adenomyosis occurs when tissue that normally lines the uterus (the endometrium) grows into the muscular wall of the uterus.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Each month, this tissue still thickens and bleeds \u2014 but trapped within muscle, it causes inflammation, pressure and pain.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The uterus can become enlarged and tender. Adenomyosis involves endometrial tissue (shown with dark splotches) growing into the uterine muscle causing painful, heavy periods and other symptoms. | Image made by author <\/figcaption><\/figure>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">Symptoms: more than &#8216;just a bad period&#8217;<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Common symptoms include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Heavy or prolonged menstrual bleeding<\/li>\n\n\n\n<li>Severe cramping or pelvic pain<\/li>\n\n\n\n<li>A feeling of pressure or fullness in the abdomen<\/li>\n\n\n\n<li>Pain during sex<\/li>\n\n\n\n<li>Ongoing fatigue<\/li>\n\n\n\n<li>Fertility challenges or infertility<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Adenomyosis vs endometriosis (what\u2019s the difference?)<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">These two conditions are often confused \u2014 and sometimes occur together.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Adenomyosis<\/strong>: tissue grows <em>within the muscle wall of the uterus<\/em><\/li>\n\n\n\n<li><strong>Endometriosis<\/strong>: similar tissue grows <em>outside the uterus<\/em> (for example, on ovaries, bowel or pelvic lining)<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Both can cause pain and fertility issues. Both are underdiagnosed.<\/p>\n\n\n\n<div class=\"wp-block-group alignfull has-background\" style=\"background-color:#ddf0fd\"><div class=\"wp-block-group__inner-container is-layout-constrained wp-block-group-is-layout-constrained\">\n<div class=\"wp-block-group\"><div class=\"wp-block-group__inner-container is-layout-constrained wp-block-group-is-layout-constrained\">\n<div class=\"wp-block-group is-vertical is-content-justification-center is-layout-flex wp-container-core-group-is-layout-524f8de7 wp-block-group-is-layout-flex\">\n<h2 class=\"wp-block-heading\">Why awareness matters<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">April is <strong>Adenomyosis Awareness Month<\/strong> \u2014 and it matters.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Too many women spend years second-guessing their own experience.<\/p>\n<\/div>\n<\/div><\/div>\n<\/div><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Why diagnosis can take time<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Adenomyosis is often misdiagnosed or dismissed. Many women were told their symptoms were <em>normal<\/em> or something to push through.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In the past, it would take about 6-8 years to diagnose. These days, it may feel slow but things are improving.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Symptoms overlap with other conditions. Imaging (like ultrasound or MRI) can help, but it\u2019s not always definitive. <\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>We have better awareness<\/li>\n\n\n\n<li>Better imaging <\/li>\n\n\n\n<li>More dedicated women&#8217;s health and endometriosis services across Australia. <\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Specialist care: gynaecologist or clinic<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">If symptoms are ongoing or complex, your GP will refer you to a <strong>gynaecologist<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This may include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Private gynaecologists<\/li>\n\n\n\n<li>Public hospital clinics<\/li>\n\n\n\n<li>Specialist pelvic pain or endometriosis clinics<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">In NSW, services may sit within hospitals under NSW Health or Central Coast Local Health District.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Do you need a referral?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Yes \u2014 in most cases.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">To see:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>A <strong>gynaecologist (specialist)<\/strong> under Medicare<\/li>\n\n\n\n<li>A <strong>public hospital clinic<\/strong><\/li>\n\n\n\n<li>Most <strong>endometriosis\/pelvic pain clinics<\/strong><\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">\ud83d\udc49 You will need a <strong>GP referral<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Without one, you usually can\u2019t access subsidised specialist care.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What should I do if I&#8217;m concerned about pelvic pain?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Make an appointment with your general practitioner.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">They can:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Take a detailed history of your symptoms (bleeding, pain, fertility concerns)<\/li>\n\n\n\n<li>Do a physical examination if appropriate<\/li>\n\n\n\n<li>Order initial tests (usually a <strong>pelvic ultrasound<\/strong>)<\/li>\n\n\n\n<li>Start first-line treatment (pain relief, hormonal options)<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Many women are <strong>first assessed and sometimes provisionally diagnosed<\/strong> by their GP based on symptoms and imaging.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">However, adenomyosis can be tricky to confirm \u2014 so GPs will often refer on.<\/p>\n\n\n\n<div class=\"wp-block-group has-background\" style=\"background-color:#e6dbe1\"><div class=\"wp-block-group__inner-container is-layout-constrained wp-block-group-is-layout-constrained\">\n<h2 class=\"wp-block-heading\">Jess has the final word<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Painful, heavy periods are common.
